

Depending on which type of DNA home test kit you purchase, your kit will contain a pre-
paid shipping envelope with a shipping label affixed to the envelope. You have the option
to select and pay for UPS or FEDEX second day, next day or Saturday delivery, or,
shipment via the United States Postal Service (USPS) using Priority Mail or Express Mail.
Most customers select second day service via UPS or FEDEX or Priority Mail (typically two
or three day service) via USPS. If you are ordering from a country other than the United
States, we will ship your DNA test kit to you via Global Priority Mail (USPS). International
customers will be required to pay for shipping to the laboratory in the U.S. separately. Feel

Your kit will also include detailed instructions regarding sample collection using the buccal
swabs that are included in the kit. A results release form will be provided for you to indicate
to the lab how you wish your test results be reported to you (phone, e-mail, hard copy via
mail, etc.). The third large sheet of paper is probably the most important form. You will
need to reveal the gender and race of each individual’s test sample. This information is
very important for accurate test results. Failure to disclose and/or accurately report this
information may render your DNA testing package inconclusive or a failed report. The
customer is ultimately responsible to provide accurate information relating to test subject’s
race and gender. A signature and date is required.
The kit will provide you with buccal swabs, and will be packaged two swabs per test
subject. Ensure you do not discard the swab’s paper wrapper, because once the swabs
are used to collect buccal cells from inside each test subject’s cheek, the swabs should be
put back into the slender sleeve wrapper.
The buccal swab sleeve wrapper will then need to be placed inside the small brown
envelope, also provided with your test kit. Ensure the information required on the outside of
the envelope is accurate and filled-in completely.
You will find a red non-tamper seal provided with each small brown envelope. Once the
buccal cells are collected on the swabs, the swabs replaced into the swab wrapper, the
swab wrapper placed into the brown envelope, seal the envelope using the red non-tamper
strip seal, affixed to the underside of each brown envelope, along the seam of the envelope.
We suggest you fill-out the information on each brown envelope, one at a time, and collect
the buccal cell samples, one at a time. This will ensure you do not forget or become
confused as to which sample goes with which envelope. Take your time and keep track of
all the paperwork.
At completion of sample-taking and the respective paperwork, place the forms and signed
and sealed brown sample envelopes in the large, pre-paid and pre-addressed shipping
envelope, seal the envelope, then take the shipping envelope to a proper shipping box or
facility. Again, this will be UPS (United Parcel Service), FEDEX (Federal Express) or
USPS (the United States Postal Service (the post office). It is very important that you place
the shipping envelope into the correct collection box because one entity WILL NOT deliver
a shipment for a different entity.
